Mailing Statement Instructions
Mailing Statement are required in order for Georgia State University faculty and staff to send outgoing mail through the USPS. To order mailing statements, please call Mail Services at 404-413-9653 or email Mail Services at firstname.lastname@example.org. There is no departmental cost.
The following are the steps involved in completing the mailing statement:
DEPARTMENT NAME/PROJECT SPEED CODE
Write in your department’s speed code – it can be any number of digits up to 13
DEPARTMENT NAME/PROJECT NAME
Write our department name and/or project name
PROJECT EXPIRATION DATE
If this speed code is for a project, please add the project expiration date.
Write in the name of the person who prepared the mailing just in case we need to contact you.
Write in your phone number just in case we need to contact you.
Please check off the type of service for item(s) you are mailing
Please indicate the type of item you are mailing.
NAME OF MAILING
Please name your mailing so Mail Services has a reference if you should have questions after we pick up your mail to be processed.
DATE SENT TO GEORGIA STATE MAIL SERVICES
Please write the date you provided the mail to the Mail Services Clerk
NUMBER OF PIECES
Please indicate the number of pieces in this mailing.
- Mailing statements are only required for mail that will need USPS postage added by Mail Services. Interoffice mailings DO NOT need a mailing statement.
- All outgoing USPS mail processed by Mail Services will be charged back to the speed code on the mailing statement.
- Please pull off the bottom (pink) copy of the three-part form to keep for you records as proof of mailing.
- Missing or incomplete statements will result in the processing being delayed. Please ask your mail clerk for more information and to obtain more statements.
