Report a Vending Machine Malfunction

Please complete the form below to request a vending refund, report a malfunction and/or low or no product.

  1. Identify machine number.
  2. Complete Vending Malfunction Report.
  3. Visit the Cashier Office on your campus (if applicable).

Please note, all refund requests over $5.00 are subject to an investigation and can take up to five (5) business days. Locate your campus refund office, bring a copy or screenshot of the confirmation page received after submitting a report. You must have a valid PantherCard or government issued ID.

To receive a refund for an incorrect vend, out of date product or product that is too hot/cold, you must be able to present the product.
