Report Vending Issues
All refund requests over $5.00 are subject to an investigation and can take up to five business days.
To request a refund, machine malfunction, product issue or under serviced machine, complete the vending issue report below.
NOTE:
- Identify vending machine number on the BLUE QR CODE Label or WHITE label.
- Visit refund location (bring government issued I.D. and the electronic confirmation page you will receive after submitting the vending issues form.)
